제목 Podocin as a Predictor of Multifocal Atherosclerosis in Diabetes Type 1 Patients and Chronic Kidney Disease

초록 Traditionally, microalbumin has been a mainstay in assessing renal function and cardiovascular risk. However, its limitations have led to the exploration of other markers, including podocin, that offer potentially improved sensitivity for detecting renal dysfunction. We aimed to evaluate the role of podocin as a potential circulating biomarker of atherosclerosis in patients with diabetes type 1 (DT1) and chronic kidney disease (CKD). A total of 155 patients with DT1, aged 34 to 64 years were examined. All patients underwent standard clinical and laboratory examination, with an assessment of the levels of the urine podocin and ultrasound examination of the brachiocephalic arteries with determination of intima-media thickness (IMT) of the common carotid artery (CCA). The thickness of the intima-media (IMT) increased with rising levels of podocin in both the right carotid artery (CA) (p < 0.001) and the left CA (p < 0.001). Levels of podocin were significantly higher in patients with involvement of two vascular beds compared to patients without atherosclerotic lesions or those with involvement of one vascular bed. A prognostic model has been developed to determine the probability of atherosclerosis developmentbased on age, duration of diabetes, HbA1c, LDL cholesterol, podocin, and the A/C ratio using binary logistic regression (the pseudo-R² = 52.9%). An increase of 1 ng/mL in podocin was associated with a 2.971-fold increase in IMT (Fig. 1). When assessing the probability of IMT thickening using ROC analysis, a cut-off point for podocin of 0.911 ng/mL was obtained, with a sensitivity and specificity of 71.0% and 72.0%, respectively. The area under the ROC curve was AUC = 0,739; 95% CI: 0,643 – 0,836, p < 0,001). Urinary podocin in urine can be considered as an early marker of atherosclerosis in patients with type 1 diabetes, with a sensitivity of 71% and specificity of 72%.
