제목 Studying the Effects of Rituximab Treatment in Pediatric Steroid-Dependent Nephrotic Syndrome in Mongolia

초록 Objectives: To assess the treatment outcomes of rituximab in children with steroid-dependent or frequently relapsing nephrotic syndrome. Methods: For Objectives 1 and 2, we included children with SDNS who were hospitalized at NCMCH between 2022 and 2025, according to predefined inclusion and exclusion criteria. • Objective 1: Case series of 6 patients treated with rituximab. • Objective 2: Case-control study. The case group included children who received six doses of rituximab at 28–30-day intervals. The control group comprised 16 children with SDNS who did not receive rituximab. Statistical analyses were performed using Microsoft Excel, IBM SPSS 27, and Review Manager 5.0. Results: No relapses were observed post-treatment. When comparing baseline and post-treatment laboratory values in rituximab-treated cases, there were statistically significant improvements in serum cholesterol (p = 0.04; 95% CI: 0.01–3.1) and albumin levels (p = 0.02; 95% CI: −3.7–0.1). There were no statistically significant differences in leukocyte, neutrophil, or lymphocyte counts in the assessment of B-cell regeneration (p = 0.102, p = 0.158, p = 0.287, respectively). In Objective 2, comparison between rituximab-treated and non-rituximab-treated groups revealed a statistically significant difference in eGFR (p = 0.04; 95% CI: 1–23) and in relapse rates (p = 0.03). Conclusion: 1. Rituximab treatment is effective in children with steroid-dependent and frequently relapsing nephrotic syndrome, as demonstrated by our clinical data. However, long-term prospective studies are warranted to assess sustained efficacy and safety. 2. In our cohort, rituximab therapy was associated with a higher glomerular filtration rate, improved overall renal function, and a reduced frequency of disease relapse compared with the control group.
