Objectives: Erythropoiesis-stimulating agent hyporesponsiveness (HypoESA) poses a significant challenge in managing anemia among dialysis patients. The lack of consensus on its definition and management creates a gap in clinical practice. This study aimed to explore nephrologists' perceptions and practices regarding HypoESA management. Methods: A 26-item cross-sectional survey was administered online to nephrologists in the Busan- Ulsan-Gyeongnam chapter of the Korean Society of Nephrology. The survey evaluated demographics, clinical experiences, perceptions of HypoESA, diagnosis, management strategies, and predictive factors. Data were analyzed descriptively. Results: Eighty-one nephrologists responded (38% response rate), revealing significant disagreement on the definition of HypoESA, with varied criteria for increases in ESA dosage. Chronic inflammation was deemed the primary cause, with C-reactive protein (CRP) identified as the key diagnostic test, while increasing iron supplementation emerged as the preferred management strategy Conclusion: Despite recognizing chronic inflammation as central to HypoESA, nephrologists demonstrated a significant lack of consensus regarding its definition and management, creating barriers to standardized care. This underscores the urgent need for a standardized definition to align clinical guidelines with practice and enhance patient care. |
