제목 Gender Disparities in Chronic Kidney Disease Burden across Southeast Asia: A Global Burden of Disease 2003–2023 Analysis

초록 Objectives: Global morbidity and death are rising due to chronic kidney disease (CKD). CKD outcomes by gender have been reported, however longterm data from Southeast Asia is few. Understanding whether discrepancies are due to disease incidence or post-diagnosis outcomes is crucial for equitable CKD management. Methods: We used Global Burden of Disease 2023 data to analyse age-standardized CKD mortality, DALYs, and incidence in nine Southeast Asian countries from 2003 to 2023. Annual percentage change (APC) measured temporal trends, while M:F ratios detected gender differences. Results: Men had a higher CKD mortality rate than women in 2023 (29.5 vs 25.5 per 100,000; M:F ratio 1.17), with notable differences among countries, such as Indonesia (1.54) and Philippines (1.51) which had substantial male excess. DALYs followed similar patterns (M:F 1.11). CKD incidence was however similar for men and women (0.96 M:F). From 2003-2023, male mortality declined (APC -0.40%/year) whereas female mortality stayed stable or increased (APC +0.13%/year), causing inequities to shrink due to male improvement. Both sexes had increased CKD incidence, suggesting expanding regional burden. Conclusion: Gender differences in mortality and disability continue in Southeast Asia despite identical CKD prevalence. The narrowing gender gap reflects unequal survival gains rather than equitable progress. These data reflect that gender-responsive CKD policies are needed to ensure equal diagnosis, treatment, and renal replacement therapy.
