제목 Clinical Profile, Microbiological Characteristics, and Antimicrobial Susceptibility of Urinary Tract Infections Among Hospitalized Patients at Dr. Hasan Sadikin General Hospital, Bandung, Indonesia (2022-2024)

초록 Objectives: Urinary tract infection (UTI) is a common cause of hospitalization and infectious morbidity worldwide, with increasing antimicrobial resistance complicating its management. This study aimed to describe the clinical profile, microbiological characteristics, antimicrobial resistance patterns, and clinical outcomes of hospitalized adult UTI patients at a tertiary referral hospital Methods: This retrospective descriptive study included adult inpatients (≥18 years) diagnosed with UTI at Dr. Hasan Sadikin General Hospital, Bandung, from January 2022 to December 2024. Medical records and microbiology laboratory data of patients with positive urine cultures were reviewed. After excluding incomplete records, negative cultures, and duplicates, 748 cases from 2,063 screened records were analyzed descriptively. Data were analyzed using Jamovi software version 2.6.44. Results: Most patients were female (69.0%) and aged 56–65 years (25.0%). Hypertension (45.3%), chronic kidney disease (31.1%), diabetes mellitus (25.4%), and malignancy (21.9%) were frequent comorbidities, and 12.8% experienced readmission due to UTI. Gram-negative bacteria predominated, with Escherichia coli (43.2%) as the leading pathogen, followed by Klebsiella pneumoniae (9.1%), Enterococcus faecalis (7.0%), Enterobacter spp. (4.1%), and Acinetobacter baumannii (3.1%). Multidrug-resistant organisms were identified in 65.7% of isolates. Escherichia coli was the most prevalent MDR pathogen, with 82 isolates in 2022, 71 in 2023, and 118 in 2024. High resistance were observed against ampicillin, third-generation cephalosporins, fluoroquinolones, and trimethoprim–sulfamethoxazole. The number of ESBL-positive cases showed a gradual increase, rising from 54 cases in 2022 to 63 cases in 2023 and sharply to 92 cases in 2024. Clinical improvement occurred in 62.2% of patients, antibiotic escalation was required in 11.9%, and in-hospital mortality reached 19.3%. Conclusion: Hospitalized UTI patients were predominantly older women with multiple comorbidities, Gram-negative predominance, a high proportion of MDR isolates, and substantial in-hospital mortality.
