제목 Heavy Alcohol Consumption on the Prevalence and Prognostic Significance of Hyponatremia

초록 Objectives: Hyponatremia is a common electrolyte imbalance frequently observed in hospitalized patients with liver disease. The independent effect of alcohol consumption on sodium imbalance remains insufficiently clarified. This study aimed to investigate the prevalence, risk factors, and prognostic impact of hyponatremia in patients with alcoholic and non-alcoholic liver diseases. Methods: We conducted a retrospective study of patients admitted to the Department of Hepatology at Gyeongsang National University Hospital between March 2016 and August 2025. Adult patients (≥18 years) hospitalized primarily for liver disease were included, excluding those with alcoholic or non-alcoholic cirrhosis. Patients were categorized into an alcoholic liver disease group (alcohol group) and a non-alcoholic liver disease group (non-alcohol group). The prevalence, clinical outcomes, and mortality associated with hyponatremia (serum sodium <135 mmol/L) were compared between groups. Results: Among 684 hospitalized patients, 201 (29.4%) were in the alcohol group and 483 (70.6%) in the non-alcohol group. Age did not differ between groups, whereas male predominance and chronic liver disease were more common in the alcohol group (both p<0.001).Hyponatremia occurred in 25.7% overall and was more frequent in the alcohol group (38.8% vs. 20.3%, p < 0.001). Heavy alcohol use independently increased the risk of hyponatremia (adjusted OR 2.21, p < 0.001). Hyponatremia was associated with higher 30-day mortality and remained an independent predictor of all-cause mortality (adjusted HR 1.71, p = 0.013). In heavy alcoholics, concomitant hyponatremia further increased mortality risk (HR 2.03, p = 0.006). Conclusion: Heavy alcohol consumption is independently associated with an increased risk of hyponatremia in patients hospitalized for liver disease. Hyponatremia significantly correlates with short-term mortality, prolonged hospitalization, and ICU admission. These findings suggest that sodium imbalance may serve as a critical mediator linking alcohol-related organ dysfunction to adverse clinical outcomes.
