A new CAPD access technique was recently developed by Moncrief to prevent peritonitis via periluminal route, A catheter is implanted in twa stages. In the first stage, the external segment of the catheter is implanted sub- cutaneously after the intra-peritoneal segment is placed intraperitoneally. In the second stage, after 6 weeks of subcutaneous implantation the external segment of the catheter is exteriorized through a small skin incision and then attached to an adaptor. This new technique apperars to effectively eliminate periluminal route of infection by allowing the tissue ingrowth around the cuff and wound healing in a sterile environment, thus reducing the incidence of peritonitis. |
